Passport Services

PLEASE NOTE: The City of Westminster has temporarily suspended its passport services. Please visit the U.S. Department of State’s website for passport information or assistance on finding a local passport office.

Westminster City Hall provides passport services for new passport applications for adults and children, and renewals for children. For adult renewals, visit the U.S. Department of State’s website .

Passport forms are available at the City of Westminster City Hall, 4800 W. 92^nd Avenue, or online at the U.S. Department of State’s website.

What to bring:

• Completed DS11 form (do not sign the form until the agent asks you to do so).
• Applicants 16 and older must have a valid, state-issued photo ID; preferably a driver's license.
• Proof of US citizenship (originals - no photocopies accepted); this includes one of:
  • State issued birth certificate filed within one year of birth with both parents names listed
  • Naturalization Certificate
  • Expired adult U.S. passport
• One passport photo - Photos can be taken at Costco, Walgreens, Walmart or FedEx. Please contact these companies directly for costs
• All applicants must be present.
  • Children 15 and under must have BOTH parents present with ID (If one parent cannot be present Form DS-3053 needs to be filled out by the non-appearing parent, notarized and accompanied by a photocopy of the front and back of that parent’s ID, or contact the passport desk at 303-658-2337 for more information.
  • Children 16-17 must have one parent present with ID.
• Payment of fees - Checks, Money Orders or Cashier’s Checks ONLY. (Please call for correct amounts. Fees are split into two payments: one to the US Dept. of State and one to The City of Westminster.)

Current processing of a new passport is four to six weeks:  Passport applications may be expedited for an additional fee. If you require your passport within two weeks, contact the National Passport Information Center at 1-877-487-2778.
